NICE issues quality standard on medicines support for individuals receiving social care

The National Institute for Health and Care Excellence (NICE) has issued an updated quality standard which focusses on assessing whether adult individuals require assistance with their medicines and determining what medicines support is required to enable them to manage their medicines.

The key goals of the quality standard are improvements in the following outcomes:

  • Carer-reported, health-related and social care-related QoL;
  • Emergency readmissions;
  • Patient safety;
  • Control over one's daily life and
  • Feelings of being supported to manage their condition.

The quality standard covers the following 4 recommendations:

  1. Individuals receiving an assessment for social care in the community should get their medicines support needs included in the assessment.
  2. Individuals receiving medicines support in the community from a social care provider should have their general practice and supplying pharmacy informed about initiation of the support.
  3. Individuals receiving medicines support in the community from a social care provider should keep a record of the medicines support required in their care plan.
  4. Individuals receiving medicines support in the community from a social care provider should be educated about raising any medicines-related problems.

Health professionals should also take into consideration the quality standards on "Transition between inpatient hospital settings and community or care home settings for adults with social care needs", "Social care for older people with multiple long-term conditions", "Home care for older people", "Medicines optimisation", "Medicines management in care homes" when providing medicines support services for people receiving social care in the community.
